How this is compared

Route to Part D if Q2, Q3 or Q4 is Yes, or if the prescription's purpose (Q4b) is CV risk reduction, or if the patient already received a GLP-1 through Part D (coverage step). Otherwise, with Q5 confirmed: Tier A is BMI 35 or more; Tier B is BMI 30 or more with HFpEF, uncontrolled hypertension or CKD 3a+; Tier C is BMI 27 or more with pre-diabetes, prior MI, prior stroke or symptomatic PAD. Tiers are not exclusive; the tool reports the highest that applies. BMI under 27 does not appear to meet the criteria. All twelve form questions are always required; there is no skip logic.
